What specific documentation must I upload to the Certification Hub to receive PDH credit for Continuing Education and Training?

When submitting an activity for Continuing Education and Training credit, you must upload supporting evidence directly into the Certification Hub. Your submission must include the following:

• A Certificate of Completion or Attendance that explicitly states:
  • The name of the individual completing the course.
  • The exact name of the course.
  • The course provider or sponsoring institution.
  • The completion date.
• A Course Syllabus or Description: You must also provide a live web link or upload supporting documentation (such as a program agenda, syllabus, or official description) that outlines the curriculum and program length. This is required so the credentialing team can verify that the course content directly aligns with functional medicine competencies and is free of commercial bias.

Note: If your original certificate or syllabus is in another language, you must upload both the original document and a translated version.
